Resale Request

We offer two methods for having your resale request completed. To complete an online resale request application, click on option #1 below. To download a resale request form, click on option #2. With either option, the fee must be included with the request. You may pay by credit card, certified check or money order only. Cash, company checks and personal checks will not be accepted. If you elect to use a credit card, a 5% convenience fee will be added to the purchase amount. Please be advised that time is calculated from the date of receipt of payment and that documents have a thirty (30) day expiration date.

Option #1
Please click below to complete the resale request form on line. All you have to do is provide the basic information on the form, click the submit button, then confirm your resale information and then click the Buy Now button which will take you to a secured card processor site to enter your credit card information. Once your payment has been accepted, the resale information will be forwarded to us for processing and you will receive a confirmation of your purchase from the card processor.

By choosing this option, you may print the request form, fill it out at your convenience and then return it to us via fax, U.S. Mail or by dropping by our office. Payments may be made via credit card, certified check or money order- the choice is yours!
